Don Black, PA is no longer practicing at CovenantCare

Physician Assistant Don Black will no longer be seeing patients at CovenantCare. Due to some pervasive health issues, he has decided to take an early retirement. We at CovenantCare are sad to see him go and wish only the best for him in his retirement.

If you are one of Don Black’s patients, we would be honored if you would continue to consider us your primary care provider. If our Sango location is convenient for you, Physician Assistant Elizabeth Rainey or Dr. David Moore are available and equipped to handle your care. If you would prefer to remain at our Hilldale location, we will have a new provider serving that location at the beginning of January, as Dr. David Johnson is booked through December.

Some Medicaid restrictions apply for seeing MDs; however, one of our Physician Assistants (PAs) or Family Nurse Practitioners (FNPs) will be able to see you. The only insurance we do not accept at this time is Cigna Connects.

CPC+ Announcement

CovenantCare has been selected as one of Clarksville's only primary care clinics to participate in an exciting and potentially game changing program from he Centers for Medicare & Medicaid Services (CMS), call the Comprehensive Primary Care Plus (CPC+).  

Strengthening primary care is critical to promoting health and reducing overall health care costs in the U.S. Building on lessons learned from the Comprehensive Primary Care (CPC) initiative and input from the 2015 Request for Information on Advanced Primary Care Model Concepts, the CMS introduced the Comprehensive Primary Care Plus (CPC+) model on April 11, 2016.  This five-year multi-payer model will begin in January 2017.

The American Academy of Pediatrics (AAP) developed a set of comprehensive health guidelines for well-child care, called Bright Futures, for clinicians to follow. Each well-child visit has an age-appropriate pre-visit questionnaire (linked below). You will notice the questions focus on developmental milestones, nutrition, safety, your child and family's emotional well-being, and recommendations from the AAP.
